|Summary Statement Forms|
Download the Word version to your own computer and then write and edit in the form (recommended). Exhibit, documentary, and performance students should bring THREE copies of the Summary Statement along with the Annotated Bibliography to the contests. History Fair judges will keep one set.
Website students must post the Summary Statement on the site along with the Annotated Bibliography. Paper students should place after the cover sheet.

The Summary Statement is required for all project categories, along with the annotated bibliography.
Students advancing to the Illinois History Expo may use the same form.

Students selected to advance to the National History Day contest will be required to rewrite their Summary Statement Form as a "process paper."
